Job Description:
Overview
The CHI Social Worker provides professional services to clients and
their families eligible for the Healthy Beginnings Services and
families on board the Mobile Health Clinic, in order to meet
identified psychosocial, emotional, financial and environmental
needs. The social worker provides psychosocial assessments,
supportive counseling, emergent crisis intervention appropriate to
setting, financial resource information, environment enhancements,
advance directive planning and referrals to community agencies for
clients and their caregivers/families in the client's home, mobile
health clinic (MHC), or other settings. Using an interdisciplinary
team approach, the social worker ensures clear communication and
helps to facilitate care transitions. The social worker identifies
and implements interventions at the individual and systemic levels
and provides expertise to high risk clients across the continuum.
The social worker works collaboratively with the multi-disciplinary
team to support the patient on a holistic level. Social Services
are provided as part of a collaboration with interdisciplinary
teams in adherence to the NFP model, and to policies, procedures,
guidelines, and standards of the Carle Health System.

This position is primarily Monday-Friday 8-5. There may be
occasional scheduled weekends when events are hosted.
This position serves the Vermilion County area. Please note
that this position is not located in Champaign/Urbana This office
is located at the Danville Riverfront Carle location.

Ability to coordinate multiple referrals concurrently and establish
priorities. Possess strong planning, teaching, and organizational
skills. Self-motivated, culturally competent, responsive, and
dependable. Ability to work effectively with diverse individuals
and groups. Ability to function at a high level of independence
while remaining an active part of the interdisciplinary home
visiting team and mobile health clinic team. Requires flexibility
in hours and job duties. Possess strong knowledge of prenatal,
early childhood, and disadvantaged populations. Possess strong
written and verbal communication skills. Ability to uphold the
Carle Behavior Standards in encounters. Provides productivity
reports as deemed necessary. Supports the growth of Health
Beginnings meet health system and community needs. Supports
policies, procedures, guidelines, and standards of NFP and of Carle
Health System. Attends and participates in bi-weekly case
conferences and team meetings, in accordance with the Healthy
Beginnings structure. Creates a culture of caring, trust, as well
as support and growth with families, community members, and team
members. Engages in continuous, organizational learning. As new
knowledge is acquired through evaluation, study, experience, and
innovative approaches, processes and initiatives are refined.
Develops and maintains knowledge base of current and applicable
federal and state laws, regulations, and accreditation standards.
Demonstrates a deep understanding and respect for the community.
Understands, supports, represents, and coaches others in the NFP
vision, mission, and model in actions and verbally to both internal
and external customers and colleagues.
Responsibilities
Essential Functions
Involves client and family in case planning decisions.
Provides social work intervention to clients and their families
eligible for the Healthy Beginnings and/or Mobile Health Clinic
services.
Helps to facilitate care transitions, referrals to social services,
and other specialist when needed.
Provides assistance and advocacy to clients in obtaining financial
resources and government entitlements.
Provides social work intervention to patient/families eligible for
the NFP Program.
Responds to referrals from healthcare team members to identify
available services for case specific needs.
Collaborates with nurse home visitors / supervisors / mobile health
clinic staff to meet the needs of high risk populations.
Details (direct or incidental) possible to enhance service/care is
communicated across service lines and among disciplines.
Documents all patient/client interactions, significant
observations, interventions, and actions taken in the client's
medical record in an appropriate and timely manner.
Formulates, develops, and implements a plan utilizing appropriate
social work interventions. Provides education and counseling to
clients and families around issues related to adaptation to the
client's illness and/or life situation. May utilize crisis
intervention, brief and long-term individual, group, and family
therapies.
Facilitates education/training modules to assist RN staff managing
basic social work needs.
As requested/required, participates in community coalition in
support of NFP as well as promotes public awareness of Healthy
Beginnings Services and the Mobile Health Clinic.
Assesses physical, emotional, social, spiritual, and environmental
needs of clients and their families as they relate to the NFP
domains.
Assists clients and their families in establishing goals and
outcomes through therapeutic relationships by providing education,
support, and referral resources.

Develops and maintains community relationships to support client
referrals.
Develops and maintains data tracking systems for Home Visiting
Services and Mobile Health Services that promote and move forward
the mission and vision of each program.
Bring innovative approaches to patient/client care to facilitate
positive outcomes.
Perform Insurance audits and intervene when necessary - sign
patients up for insurance.

Compensation and Benefits
The compensation range for this position is $27.36per hour -
$45.69per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate's
experience, qualifications, location, training, licenses, shifts
