RN - Healthy Beginnings at Carle Health summary: The RN Healthy
Beginnings provides comprehensive nursing care through home visits
to women and families enrolled in the Nurse-Family Partnership
Program. This role requires coordinating care, assessing physical
and psychosocial needs, and supporting client goals while adhering
to clinical standards and NFP guidelines. It involves collaboration
with community resources and team members to promote health
outcomes and program awareness. Overview $5,000 Sign on Bonus
Available $2,500 Relocation Bonus available (greater than 50 miles)
(external candidates only) The RN Healthy Beginnings is a
professional caregiver who is a member of the interdisciplinary
team and is responsible for providing comprehensive nursing
services to women and their families eligible for the Nurse-Family
Partnership Program(NFP). Nurse home visitors are responsible for
maintaining the highest standards in clinical nursing practice and
adherence to the NFP model, and to policies, procedures,
guidelines, and standards of NFP and of the Carle Health System.
Additional responsibilities include the coordination and provision
of direct and indirect patient care using the nursing process to
meet the physical, psychosocial, environmental, and spiritual needs
of Healthy Beginnings specific patient populations and families
throughout the geographical area. Qualifications Certifications:
Proof of Auto Insurance - Varies Driver's License - Secretary of
State (SOS)Secretary of State (SOS); Licensed Registered
Professional Nurse (RN) - Illinois Department of Financial and
Professional Regulation (IDFPR)Illinois Department of Financial and
Professional Regulation (IDFPR); Certified Lactation Counselor
(CLC) within 2 years - Academy of Lactation Policy and Practice
(ALPP)Academy of Lactation Policy and Practice (ALPP); Basic Life
Support (BLS) within 30 days - American Heart Association
(AHA)American Heart Association (AHA), Education: Associates
Degree: Nursing (Required); College Diploma: Nursing (Required),
Work Experience:2yrs of nursing Responsibilities Provides home
visits to women and their families eligible for the NFP Program.
Coordinates the care/services for each client; carries a case load
of up to 25 clients. As requested/required, participates in
community coalition in support of NFP as well as promotes public
awareness of NFP and the Healthy Beginnings program. Develops and
maintains community relationships to support client referrals.
Follows nursing process in implementing NFP into nursing practice;
Develops therapeutic relationships, utilize concepts of reflection
and motivational interviewing with women and their families in a
home visiting environment. Assesses physical, emotional, social,
spiritual, and environmental needs of women and their families as
they relate to the NFP domains. Assists women and their families in
establishing goals and outcomes; Provides education, support and
referral resources in assisting women and their families in
attaining their targeted goals. Participates in review and analysis
of reports for achievements and areas for improvement as well as
quality improvement efforts. Formulates nursing diagnosis based on
nursing assessment and client goals and then evaluates client
progress toward stated goals and NFP outcomes. Plans home visits as
well as joint home visits with Healthy Beginnings Nurse Supervisor
in accordance with client goal and NFP outcomes and expectations.
Consults and collaborates with other professionals involved in
providing services to women and families. Understands, supports,

model in actions and verbally to both internal and external

this position is $30.39per hour - $52.27per hour. This represents a
good faith minimum and maximum range for the role at the time of
posting by Carle Health. The actual compensation offered a
candidate will be dependent on a variety of factors including, but
not limited to, the candidate’s experience, qualifications,
location, training, licenses, shifts worked and compensation model.
