The Visit Nurse will be responsible for providing compassionate care to patients with catastrophic injuries, evaluating care options, and working with caregivers to ensure the well-being of patients. This role is based in Chicago, Will, Cook counties. Qualifications

• Patient assessment and care skills
• Knowledge of long-term care and home care practices
• Effective communication and interpersonal skills
• Ability to work collaboratively with a team
• Experience in working with catastrophic injury patients
• Current RN license in the state of Illinois
• Bachelor's degree in Nursing is plus
