FTE: 0.900000Shift: VariesLocation: Riverside Medical CenterSummary:The RN Admission & Referral Specialist will be responsible for direct contact with clients, referral sources, families, physicians and treatment teams. The RN Admission & Referral Specialist will be expected to complete an assessment of the patients in coordination with the centralized intake teamand emergency department. Will coordinate disposition with Psychiatrist and ED Physician (if indicated) and follow the patient through admission to inpatient. The RN Admission & Referral Specialist will be responsible for all ages pediatric to geriatric. Must be able to adapt quickly, work independently, detail oriented and organized. The RN Admission & Referral Specialist will review all incoming referral packets for admission criteria to one of the mental health units.Experience/Education Requirements:Graduate of an approved school of nursing, BSN preferred. Must complete BSN within 4 years of hire.Evaluation, treatment, and referral of Behavioral Health patients preferredExperience with EPIC and workque preferred.Excellent written, verbal and organizational skills.License or Certification Requirements:Current Illinois RN licenseCurrent CPR certification or obtainment within 6 months of hire with subsequent recertification annually.Psychiatric Nursing Certification preferredResponsibilities:Ability to evaluate and review clinical information and triage.Demonstrates an understanding of the action and side effects of psychotropic medications.Develops and maintains relationships within the multidisciplinary team.Functions as a resource person to patient and familiesWork collaboratively with departments, patients, family, and physician referral.Demonstrates knowledge and skill of the proper completion of involuntary petition and certificate for admission.Possesses knowledge of DSM V diagnoses and related symptoms, and demonstrates skill in identifying and assessing for theseAbility to present and document clinical impressions efficiently, and collaborate with physicians for dispositionWork collaboratively with ED, units, and other departments to facilitate assessment and admission of Behavioral Health patientsDemonstrates ability to educate patients, families and collaterals about the admission process and Behavioral Health servicesAbility to complete quality clinical documentation in a timely manner and according to documentation requirements and standardsAbility to manage multiple tasks effectively in a fast-paced environmentAbility to manage the Intake workque effectivelyDemonstrates crisis intervention and de-escalation skills with Behavioral Health patients and familiesServe as liaison across units, departments, and the community to promote access and continuity of care for Behavioral Health patientsRiverside Healthcare is revolutionizing care using leading-edge technology to diagnose and treat patients.
