Overview
$5,000 sign-on bonus and $5,000 relocation (for greater than 100
miles) or $2,500 relocation (for greater than 50 miles) for nurses
with 1 or more years experience - External Applicants Only

The Registered Nurse (RN) who serves in the role of System Case
Management Specialist/Analyst will serve the Case Management
division in orientation and onboarding, competency management,
education, role development, building collaborative partnerships
with the goal of establishing evidence based best practice with
quality outcomes. The Case Management Specialist/Analyst collects
and analyzes data; allowing for a data driven approach to identify
strategies and resources needed to achieve expected goals. Helps
implement projects, programs, and processes for teams within the
Case Management Division. Applies practical knowledge of Case/Care
Management to administer excellence with policies, procedures, and
plans. The ideal candidate will have a strong analytical
background, excellent communication skills, and a passion for
improving patient care outcomes. The incumbent will have a deep
understanding of utilization review and case management principles,
case management best practices, policies, and guidelines. Proven
leadership abilities and professional accountability through
effective communication and relationship-building are pivotal to
the success of the incumbent.
Qualifications
License/Certifications:
Licensed Registered Professional Nurse (RN) - Illinois Department
of Financial and Professional Regulation (IDFPR); Certified Case
Manager within 2 years - Commission for Case Manager Certification
(CCMC)
Education:
Bachelor's Degree: Nursing
Other Knowledge/Skills:
Case management experience 2+ years
Responsibilities
Maintain current knowledge of case management and utilization
review trends including the entire continuum of care within and
beyond hospital walls.Design and continuously improve/adapt
curriculum depending on educational needs.Develop education
strategies incorporating evidence-based research into educational
plans, patient discharge and care transition plans including
nursing practices.Analyze data to identify trends, patterns, and
opportunities for improvement in patient care managementServes as a
mentor and resource to case management team members, identifying
growth opportunities and educational needs.Promote critical
clinical thinking among case management team members and show
commitment to professional development.Develop and implement a
standardized approach to onboarding and orientationDevelop, review,
and update all case management competency documents.Work closely
with Case Management Regional leadership and Human Resources to
ensure efficient onboarding of new team members.In collaboration
with Case Management Regional leadership, develop an annual
education plan.Participate in recruitment, orientation, and
retention strategies.Participate in identifying new technology and
provide leadership for the implementation of new IT
technology.Maintain data analysis, providing insight into critical
benchmarks such as length of stay, readmissions, post-acute care
metrics, etc..

Compensation and Benefits
The compensation range for this position is $34.66per hour -
$59.62per hour. This represents a good faith minimum and maximum
range for the role at the time of posting by Carle Health. The
actual compensation offered a candidate will be dependent on a
variety of factors including, but not limited to, the candidate's
experience, qualifications, location, training, licenses, shifts
